Do the cast of the film come to see all the screenings for their film or do they only attend the premium/gala event screenings?

You'll have a far better chance meeting them/have a conversation with them at a regular screening, preferably if at the Scotiabank or at the AMC. They might show up at the Varsity as well, but don't count on seeing any of them making a special appearance at the VISA ELGIN THEATRE/Winter Garden for a repeat, chances are slim to none.
Plus, unless you're sitting at the mezzanine center at Roy Thomson Hall (by far the worst venue), which requires a lot of donated money and or connections, chances are you won't get to see a celebrity from at least 200 feet away... If you're there just for the celebs, just know where the after parties are (especially the InStyle Hollywood Foreign Press party on the Tuesday night) - they should be posting a full list of those and where they will be sometime this week - and/or try catching an immediate repeat of a film (less than 2 days apart) at the Scotiabank or at the AMC. The chances they show up at those are 80-100% certain (from my past experiences).
But most of all, just have a blast!

Thanks for replying :) Very helpful answer. I'm going for the movies mainly but the celebs come with that :) so how do I find this list?

The stars are in town to promote their films: at press conference, at interviews with the print/TV/Internet media, and at the screenings. They will often show up for regular screenings--the two main exceptions being if the screenings are spaced far apart (most stars won't remain in town for the whole week, so try to go to the early screenings) and the early morning screenings the day after an evening gala (and after-party). While the director may sometimes show up, the stars usually will not.
Also keep in mind that some non-premium films will still have stars. I think I read that Maria Bello and Michael Sheen will be in town for Beautiful Boy, for example, which has a non-premium debut at the Isabel Bader (a good theatre for seeing the stars up close).
Check TIFF's list of expected guests (http://www.tiff.net/press/pressreleases/2010/toronto-international-film-festival-announces-hundreds-of-guests-for-2010-event) and Internet reports of celebrity sightings.
Also, some stars will even show up to watch films as audience members. Last year I saw the lovely Patricia Clarkson sitting in the audience at the screening of A Single Man (at the Isabel Bader). Michael Moore is also known to go to screenings as an audience member.

I had seen Sam Neil in the audience once and I bumped into Atom Egoyan as we both were picking our preferred seats before the movie started.
